Welcome to Dr. Kiran C. Patel High School (PHS) ! We are excited that your child is enrolling in our school, and want to make the process as easy as possible. Listed below you will find information for the registration process. 
Students are officially  enrolled once the student’s registration packet has been filled out in full and received and confirmed by PHS on or before the paperwork deadline.  As part of the registration process, please complete and submit the information requested in this online student registration form. You will need to fill out an online registration form for each student that is enrolling in PHS. You can expect to spend approximately 10 minutes on this process for each student. The information you enter can be saved so you may return to the form to complete it at a later time, if needed.

NOTE: YOU MUST CLICK "SUBMIT TO SCHOOL" TO COMPLETE THIS ONLINE STUDENT REGISTRATION FORM. THE REGISTRATION PROCESS WILL NOT BE FINISHED UNTIL THE SCHOOL HAS RECEIVED ALL THE REQUIRED DOCUMENTS.
