Once you've accepted your seat, the next step is updating and verifying all sections of your application.
Please follow these instructions:
1) Ensure all information is up to date under the following headers
⦁ Primary household, parent/guardian, and emergency contact (click save/continue)
2) Under the student section, ONLY enter the child who has been offered a seat and complete the document uploads (details found below)
3) Review your completed application and submit your registration
The following is a brief summary of documentation a student will need to be officially enrolled at Amana West:
1) Complete OLR student registration
2) Under STUDENT SERVICES, most recent copy of any paperwork regarding (IEP, 504, Gifted)
3) Under PREVIOUS SCHOOL, copy of most recent Report Card (NOT REQUIRED FOR K)
4) Under DOCUMENT UPLOADS
⦁ Birth Certificate/Passport
⦁ Copy of student's SS card/number
⦁ Current Certificate of Immunization (Georgia Form 3231)
⦁ Current Hearing, Dental, Vision Screening Certificate (Georgia Form 3300)
⦁ TWO current forms of Proof of Residence (appropriate records for verification of residency must include any two of the following in the name of the parent/guardian):
*One (1) current Utility Bill (Water OR Electric)
*One (1) additional current proof of residence (ex: Home Mortgage Bill, Valid State Driver's License/ID, Home Sales Contract, Section 8/HUD Housing Contract, Homeowners/Renter's Insurance Card, Bank Statement, Paycheck Stub, Apartment/House Lease, HOA Bill/Statement)
After submitting your registration, your application will be processed and any outstanding updates will be communicated via email.
